Terms and Conditions
Thank you for taking part in the TIC Health survey.
Everyone who completes the survey and provides the required contact details can choose to be entered into our prize draw for the chance to win a £150 Amazon voucher.
Please read the terms and conditions below for full details on eligibility, how the prize draw works, and how the winner will be selected and contacted.
By entering the TIC Health Survey Prize Draw, you agree to the following terms and conditions.
1. Promoter
The promoter is TIC Health Limited, registered in England and Wales under company number 11814996, with its registered address at 1 Kings Avenue, London, N21 3NA.
2. Eligibility
The prize draw is open to UK residents aged 18 or over.
Employees of TIC Health Limited, their immediate family members, and anyone directly involved in the administration of the prize draw are not eligible to enter.
No purchase or payment is necessary to enter.
3. How to enter
To enter the prize draw, participants must:
- Complete the TIC Health healthcare membership survey; and
- Provide the requested contact details for entry into the prize draw.
Only one entry per person is permitted.
There is no charge to enter the prize draw.
4. Closing date
The prize draw closes at 11:59pm on 13th September 2026.
Entries received after the closing date will not be included in the prize draw.
5. The prize
There is one prize of a £150 Amazon.co.uk Gift Card.
The prize is non-transferable and no cash alternative will be offered.
TIC Health reserves the right to substitute the prize with an alternative of equal or greater value if circumstances outside its reasonable control make this necessary.
6. Winner selection
One winner will be selected at random from all valid entries received before the closing date. The draw will take place within 7 days of the closing date using a random selection process.
7. Contacting the winner
The winner will be contacted using the email address or telephone number provided when entering the prize draw.
TIC Health will make reasonable efforts to contact the winner. The winner must respond within 5 days of being contacted to claim their prize.
If the winner does not respond within this period, is found to be ineligible, or is otherwise unable to accept the prize, TIC Health reserves the right to select another winner at random from the remaining valid entries.
The prize will be provided to the winner within 30 days of successfully claiming it.
8. Personal information
Personal information provided for the prize draw will be used to administer the draw, contact the winner and fulfil the prize.
Any personal information collected will be handled in accordance with TIC Health’s Privacy Policy and applicable UK data protection law.
9. General
TIC Health reserves the right to verify the eligibility of any entrant and to disqualify entries that it reasonably believes are fraudulent, incomplete or otherwise in breach of these terms and conditions.
TIC Health accepts no responsibility for entries that are lost, delayed, incomplete or not successfully submitted due to a technical fault or circumstances outside its reasonable control.
TIC Health reserves the right to cancel, suspend or amend the prize draw where it becomes necessary to do so due to circumstances outside its reasonable control.
The prize draw is not sponsored, endorsed, administered by or associated with Amazon. Amazon is a trademark of Amazon.com, Inc. or its affiliates.
These terms and conditions are governed by the laws of England and Wales, and entrants will be subject to the jurisdiction of the courts of England and Wales.
